By signing this Shidduch Application Form, you (the “Applicant”) hereby acknowledge that Congregation Beth Shalom of Lawrence, Young Israel of Lawrence-Cedarhurst and Young Israel of Woodmere, and their respective officers, directors, trustees, members and representatives (collectively, the “Sponsor”) make no representations or warranties, express or implied, about the suitability of any potential or actual Shidduch match introduced to the Applicant by the Sponsor. The Applicant also acknowledges that he or she must provide detailed, current and accurate personal information to the Sponsor in order for the Sponsor to properly present the Applicant to potential Shidduch matches.
